Due to the VA/CMC SOS quality assurance protocols, new VA/CMC SOS sites must be created by the VA SOS coordinator. If you would like to begin monitoring at a new site, report it to the coordinator using the forms below. The coordinator will reach out to you via email once your site has been created.
